The qualifiers
In Washington State, the Foundational Community Supports (FCS) program is available to eligible individuals who meet certain criteria. Here are the general qualifications for FCS in Washington State:
Medicaid Eligibility: To qualify for FCS, individuals must be enrolled in Washington State's Medicaid program, known as Apple Health.
High Need and Complexity: FCS is intended for individuals with high needs and complex challenges that impact their overall well-being. This includes individuals with mental health conditions, substance use disorders, chronic health conditions, or other factors that contribute to their vulnerability.
Risk factors: FCS targets individuals with complex needs and risk factors such as homelessness, unemployment, behavioral health challenges, and chronic health conditions to provide comprehensive care coordination and supportive services.
